What goes on a testing slip

What information should a student testing slip include?

At minimum each slip needs the student's name, the class period or block, the test name, and the test ID or code that identifies the version they're taking. Including the version lets a proctor confirm a student has the correct paper, and adding accommodations (extra time, small group, and so on) tells the proctor how to support that student during the test.

What's the difference between a test version and a test ID?

A test version (often labelled Form A, Form B, or Form C) is the specific printed paper a student receives — teachers hand out different versions so nearby students can't copy answers. The test ID is the numeric or alphanumeric code printed on that paper. On the slip, the version tells you which form to give out and the test ID lets you verify it against the master list before testing begins.

Assigning test versions

How do I assign different test versions so students can't share answers?

Assign each student one of your test versions (A, B, or C) so that students sitting near each other receive different forms. With Test Slip Maker you pick a version per student from a dropdown, or use "Set all" to assign one version to the whole class and then change the few who need a different form. The slip prints the version and its test ID so you can match each student to the correct paper.

Can a student have more than one test code on their slip?

Yes. Some students take multiple sections, retakes, or modified forms. Each student can carry up to three test codes, and the slip lists each version with its test ID and test name on its own line. This is handy when a student has a regular form and a modified form, or sits for more than one testing session.

How do I reuse the same student list for a new test?

Keep your roster saved and change only the test name and test ID each time. Upload your roster once, then use "New test, keep roster" to clear the old version assignments while keeping every student. You can also save named sets of students-plus-tests and load them back for recurring assessments.

Accommodations

What accommodations can I print on a testing slip?

Common testing accommodations include extra time, small group, read-aloud, headphones, supplemental aids, sentence stems, modified choices, and language supports. On each slip you can tick the accommodations box and choose from preset options or add your own; the selected accommodations print on the ticket so a proctor sees them at a glance.

How do I show a substitute or proctor what each student needs?

Print a proctor class list alongside the slips. The class list shows every student's name, class block, test version, test ID, and accommodations in a single table a proctor can read quickly. You can also print a separate accommodations list that groups students by each accommodation (for example, everyone who gets extra time) and then by class period.

How do I group students by accommodation for testing?

Use the Accommodations print view. It lists each accommodation (Extra time, Small group, etc.) as its own section, and within each section splits students by class period. A student with two accommodations appears in both groups, and students with no accommodations are left off so the list stays focused on the students who need support.

Roster management

Can I import my student roster from a spreadsheet instead of typing it?

Yes. Upload a .csv, .xlsx, or .xls file and Test Slip Maker reads the student names and class periods automatically. If your workbook has several sheets, it picks the one with the most students. You can also import test version assignments straight from a spreadsheet that has version, test ID, and test name columns.

How do I filter or sort students when assigning versions?

Use the filter bar above the student list: search by name, filter by class period, or show only students who have a given test slot assigned. A Sort by dropdown orders the list by name (A to Z) or by class period. These filters help you find a student quickly in a large roster.

Printing

How do I print multiple testing slips on one page to save paper?

Choose 4, 6, or 8 slips per page and the slips arrange in two columns with dashed cut lines between them. The layout is sized for US Letter paper with no margins, so when you print or save a PDF each slip lines up for cutting. Use your browser's Print dialog and choose "Save as PDF" if you'd rather keep a digital copy.

How do I print a proctor class list grouped by period or test version?

In Class list options, pick whether to print one list for everyone together, a separate list per class period, or a separate list per test version. You can also toggle which columns appear (name, block, test version, test ID, test name, accommodations) so the handout has exactly what your proctors need.

How do I keep track of which slips I've already printed?

Use the printed slips log. Click a student's slip to mark it printed (with a timestamp), or mark all as printed after a run. A progress bar shows how many are done and how many remain, and you can show only the unprinted slips the next time so you don't hand out duplicates.

Slip layout

Can I lay out the slip to match my actual printed ticket?

Yes. The template editor lets you reorder the lines on the slip by dragging, choose whether the student name and class block sit stacked or side by side, adjust the label and test ID widths, change the font size, and switch borders between dashed, solid, or none. The same order carries over to the proctor class list.

How do I reduce toner use when printing lots of slips?

Pick the "none" or "dashed" border style and avoid heavy fills. The slip layout is designed to print cleanly in black on white, so you can run a full class set on a standard printer without using large amounts of toner.
